Designed specifically for patients who have undergone weight loss surgery, our program offers personalized, long-term follow-up care to ensure you reach your goals and maintain a healthy lifestyle for years to come.

AfterCare Program Highlights
Our AfterCare Program offers flexibility and customization to meet your individual needs:
Ongoing Support: Enroll annually and continue your program for as long as you need, with regular virtual appointments to help guide you through your post-surgery journey.
Personalized Care: Work one-on-one with a bariatric nutrition educator who will provide judgement-free, personalized advice to support your health, dietary, and lifestyle changes.
Educational Resources: Access a wealth of educational content focused on nutrition, behavior changes, and other key topics critical to post-surgery success.
Tailor the program to your specific needs, with additional services available (fees may apply).
Program Details
There are three AfterCare packages available, depending on your surgery date and where you have had surgery.
Current Bariatric Patients
Patients undergoing Bariatric Surgery at Surgical Solutions Network and SmartShape Weight Loss Centre BEFORE November 1st, 2024.
New Bariatric Patients
Patients undergoing Bariatric Surgery at Surgical Solutions Network and SmartShape Weight Loss Centre AFTER November 1st, 2024.
All Other Bariatric Patients
Patients undergoing Bariatric Surgery NOT at Surgical Solutions Network and SmartShape Weight Loss Centre.
